TERMS AND CONDITIONS/MEMBER HEALTH AGREEMENT DECLARATION
You warrant, declare and acknowledge that:

The information given by you in entering this agreement is correct and will be relied upon by us.

Our staff are not medically trained, and should you have any concerns with your health and fitness you should seek independent medical advice before engaging in any physical activity on our premises.

To the best of your knowledge and belief you are in good health and not knowingly incapable of engaging in either active or passive exercise and that such exercise would not be detrimental to your health, safety, comfort, well being or physical condition. Further, that you will advise us immediately should your health or vulnerability to injury change.

You have read and understood this agreement and all of its the Terms and Conditions before accepting them.

You agree that your doctor has never advised you of a heart condition and are not currently on medication for a heart or blood pressure related condition.

You agree that you have got approval from your doctor stating that you are fit and well enough to take part in physical activity in a gym environment.

To cancel your membership: Please cancel your direct debit with your bank. You need to do this no later than 4 days before your next payment date to stop that payment from coming out.

We are not obliged to give a refund if for whatever reason you do not come to the gym after purchasing an open gym or group training membership.

If you miss any direct debit payments, you will have to pay the outstanding balance.

Any programme deposits paid are to secure your limited spot on the programme and are non-refundable.

We reserve the right to close the gym for a temporary period of time for any reason (for example: maintenance/refurb work).

From time to time we may need to increase membership price. We will give you notice before any price increase and will make it clear when the price increase will take effect and how much your membership will cost after the increase. During this period, you will have your usual right to terminate your membership in accordance with the membership terms and conditions and rules. If you do not terminate the membership by the date given to you in the notice, then the price of your membership will be increased in accordance with our notice.

Membership will be a one off annual payment for the price offered at the time or subject to a 12 month repayment with interest (monthly direct debit).
You are primarily responsible for your health and well being, but we at Treadmill Gym are concerned that you enjoy our facilities safely. To that end expect the following:

You will seek advice from a member of staff if you are unsure about using any of our equipment provided on site.

You will respect the equipment that you use on site, always behave in an appropriate manner when using our facilities and tidy up after yourself. If we ask you to leave the gym due to not following these rules, your membership will be automatically cancelled without refund.

We respect your decisions and allow you to make your own decision on your exercise regime however we reserve the right to ask you not to exercise beyond your capability.
